ABSTRACT

Introducción: A pesar del gran avance técnico que representan las fístulas arterio-venosas internas para la hemodiálisis, estas no están exentas de complicaciones que comprometen su durabilidad. Entre ellas se encuentran los aneurismas venosos yuxta-anastomóticos. Objetivo: Presentar el tratamiento quirúrgico utilizado para reparar los aneurismas venosos yuxta-anastomóticos. Presentación del caso: Paciente de 54 años, de género femenino y color de piel negra. Presentó antecedentes de padecer insuficiencia renal crónica de 12 años de evolución, con transplante renal fallido, la cual tuvo un aneurisma venoso yuxta-anastomótico, localizado en una fístula arterio-venosa húmero-cefálica en miembro superior izquierdo, que fue confirmado por eco-doppler. Se emplearon como tratamiento quirúrgico la aneurismectomía y el injerto por sustitución con prótesis vascular expandible de poli-tetrafluoretileno. Finalmente, se preservó la fístula arterio-venosa, así como su permeabilidad. Conclusiones: La evolución fue satisfactoria en cuanto a su durabilidad y utilización como vía de acceso para la hemodiálisis(AU)


Introduction: Despite the great technical advance represented by internal arterio-venous fistulas for hemodialysis, these are not exempt from complications that compromise their durability. Among them are juxtaanastomotic venous aneurysms. Objective: Present the surgical treatment used to repair juxtaanastomotic venous aneurysms. Case presentation: 54-year-old patient, female and black skin color. She presented a history of chronic renal failure of 12 years of evolution, with failed kidney transplant, which had a juxtaanastomotic venous aneurysm, located in a humerus-cephalic arterio-venous fistula in the left upper limb, which was confirmed by Doppler echo. Aneurysmectomy and replacement grafting with expandable polytetrafluorethylene vascular prostheses were used as surgical treatment. Finally, the arterio-venous fistula was preserved, as well as its permeability. Conclusions: The evolution was satisfactory in terms of durability and use as an access route for hemodialysis(AU)

Introducción: La insuficiencia renal crónica es una enfermedad frecuente que requiere del uso vital de la hemodiálisis, procedimiento común en los pacientes que la padecen, por lo cual resulta necesario disponer de un acceso vascular adecuado. Objetivo: Caracterizar a los pacientes con fístulas arterio-venosas para hemodiálisis en el Hospital Clínico-Quirúrgico "General Freyre de Andrade". Métodos: Se realizó un estudio descriptivo-retrospectivo en 88 pacientes con fístulas arterio-venosas. Se recogieron las siguientes variables: edad, sexo, tipo de acceso vascular, localización, complicaciones inmediatas y tardías, y procedimiento corrector. El período de estudio abarcó desde enero de 2018 hasta enero de 2019. Resultados: Se les realizaron a los pacientes un total de 88 fístulas arterio-venosas. Predominó el sexo masculino (67 por ciento). Prevaleció el grupo de edades de 50 a 59 años (36,4 por ciento). La fístula arterio-venosa humero-cefálica (48,9 por ciento) resultó el procedimiento más realizado. El hematoma, la trombosis y la infección fueron las complicaciones inmediatas con mayor frecuencia (3,4 por ciento). De las complicaciones tardías, predominó la trombosis (3,4 por ciento), mientras que la trombectomía prevaleció como procedimiento corrector (37,5 por ciento). Conclusiones: Como parte del acceso vascular, la fístula humero-cefálica presentó los mejores resultados en cuanto a permeabilidad y durabilidad. Por otra parte, la fístula humero-humeral constituyó una alternativa cuando no se pudieron usar las venas cefálica y basílica a nivel del pliegue del codo(AU)


Introduction: Chronic kidney disease is a frequent condition that demands the vital use of hemodialysis, a common procedure in patients who suffer from it; therefore, it is necessary to have adequate vascular access. Objective: To characterize patients with arteriovenous fistulas for hemodialysis at General Freyre de Andrade Clinical-Surgical Hospital. Methods: A descriptive-retrospective study was carried out in 88 patients with arteriovenous fistulas. The following variables were selected: age, sex, type of vascular access, location, immediate and late complications, and corrective procedure. The study period covered from January 2018 to January 2019. Results: A total of 88 arteriovenous fistulas were approached among all the patients. The male sex predominated (67 percent). The age group 50-59 years (36.4 percent) prevailed. The procedure for creating a humeral cephalic arteriovenous fistula (48.9 percent) was the most performed. Hematoma, thrombosis and infection were the immediate complications with the highest frequency (3.4 percent). Among late complications, thrombosis prevailed (3.4 percent); while thrombectomy prevailed as a corrective procedure (37.5 percent). Conclusions: As part of vascular access, the procedure for creating humeral cephalic fistula presented the best outcome in terms of patency and durability. On the other hand, the procedure for creating the humeral fistula was an alternative when the cephalic and basilic veins could not be used at the level of the elbow crease(AU)

ABSTRACT

Se presenta el caso de un paciente joven con fístula arterio-venosa congénita en miembro superior derecho, que evoluciona con elefantiasis del miembro afectado, miocardiopatía dilatada e insuficiencia cardíaca de alto gasto. Ante el fracaso del tratamiento quirúrgico (ligadura de la fístula), se decide la embolización percutánea de la misma.


We present a case of a young patient with congenital arteriovenous fistula in the right upper limb, which evolves with elephantiasis of the affected limb, dilated cardiomyopathy and high output heart failure. With the surgical treatment failure (fistula ligation), we decided the fistulas embolization.


Relatamos o caso de um jovem paciente com fístula artério-venosa congênita no membro superior direito, que evolui com elefantíase do membro afetado, cardiomiopatia dilatada e insuficiência cardíaca de alto débito. Com o fracasso do tratamento cirúrgico (ligadura de fistula), decidiu-se a embolização da fístula.

Materiais e métodos: Foram analisados retrospectivamente dados epidemiológicos, clínicos elaboratoriais de 246 pacientes com DRC encaminhados para TRS entre janeiro de 2004 e janeiro de 2006. Resultados: 50,6% eram homens, com54,5±15,5 anos de idade e tempo médio de acompanhamento de 24,8+34,5 meses. As principais causas de DRC foram: diabetes (DM-32,9%), hipertensãoarterial (HAS-25,2%) e glomerulonefrite crônica (GNC-13%). Apenas 20,6% apresentavam fistula artério-venosa (FAV) funcionante. Apenas três pacientes(1,2%) foram encaminhados a programa de diálise peritoneal (DP). Os homens tiveram chance 2,3 vezes maior de ter FAV confeccionada em relação àsmulheres (p=0,012 IC 1,2-4,4). Nos pacientes com mais de 12 meses de acompanhamento, havia chance 4,6 vezes maior para a presença de FAV(p=<0,0001 e IC 2,1-10,0). A doença com maior prevalência de FAV confeccionada foi doença renal policística (31,8%) e a com menor, DM (13,4%). Ospacientes diabéticos apresentaram risco 2,3 vezes maior de ter tempo de seguimento ambulatorial inferior a 12 meses (p=0,0027 e IC 1,3-3,9) e risco de2,3 vezes maior de não ter FAV confeccionada (p=0,029 e IC 1,1-4,9). Conclusão: Os dados analisados assemelham-se à estatística norte-americana emrelação ao DM como causa principal de DRC. O baixo encaminhamento para DP sugere dificuldade do sistema para absorver pacientes nesta modalidadee/ou tendência preferencial de encaminhamento para hemodiálise. Dificuldades locais concernentes à confecção de FAV refletem-se na baixa prevalênciade acessos funcionantes por ocasião do encaminhamento (20,6%), principalmente quando o tempo de seguimento é inferior a 12 meses, o sexo é ofeminino e a doença de base é DM.


Objectives: to analyze the profile of chronic kidney disease (CKD) patients before starting renal replacement therapy (RRT), from a tertiary nephrologycenter. Materials and Methods: we analyzed retrospectively, epidemiologic, clinical and laboratory data from 246 CKD patients assigned to RRT, fromJanuary 2004 to January 2006. Results: In this study, 50.6% were male. The main etiologies of CKD were: diabetes (DM-32.9%), hypertension (HTN-25.2%), and chronic glomerulonephritis (CGN-13%). The median age and follow-up time were 54.5+15.5 years and 24.8+34.5 months, respectively. Only20.6% patients had a patent arteriovenous fistula (AVF). Only 3 patients were assigned to a peritoneal dialysis program (PDP). After analysis, men had 2.3times greater chance of AVF through vascular access, than women (p=0.012, CI 1.2-4.4). However, for those with more than 12 months of follow-up, ahigher probability (4.6 times) of AVF was detected (p<0.0001, CI 2.1-10.0). PKD patients had the best scores for AVF placements (31.8%) and patients withdiabetes revealed the worst scores (13.4%). CGN presented the longest ambulatory follow-up time before starting RRT, whereas obstructive nephropathypresented the smallest. There was no difference between gender distribution and creatinine clearance in patients with and without diabetes. However,diabetic patients were 2.3 times more prone to have a follow-up time less than 12 months (p=0.0027, CI 1.3-3.9) and 2.3 times less prone to have anaccessible AVF (p=0.029, CI 1.1-4.9). Conclusions: The analyzed data in this tertiary hospital are in tandem with the North-American statistics, which referto diabetes as the main cause of CKD. The low number of patients assigned to a PDP suggests that the structure and organization of peritoneal dialysisfacilities cannot meet the demand and also suggests a preference towards hemodialysis as the mode of choice for RRT...
